Declaration of Romantic Intent
I love forms and extraneous verbiage, so I love The Bureau of Communication. My current favorite form is the Declaration of Romantic Intent, which you can fill out, customize and email to your special someone. Here's the one I wrote:

I can't say I'd recommend it for people who aren't already dating (if I got one of these from someone I only vaguely knew, I would probably run away screaming), but it's a fun way to say "hey, I am thinking of you and LOOK! I GOT TO FILL OUT A FORM".
